Why do I need to complete Form W-9?

United States Federal regulations require We Are One Designs to report to the Internal Revenue Service (IRS) rebate payments made to "Customers". We Are One Designs reports these payments to the IRS via Form 1099 Misc.

 

In order to comply with IRS regulations and complete the rebate process, we must have a tax identification number (TIN) or Social Security Number (SSN) for each U.S. Customer. Please read all of your notification materials carefully, as there may be other taxes that you could be responsible for. 

 

What do I do next?

  1. Download and fill out the current W-9 form please click here
  2. Sign, make a copy for yourself, and mail back to our address:
    • Attn:  Fundraising Department

  3. You will have 30 days to send back to We Are One Designs

What happens if I don’t send back a W-9 form within 30 days?

If we do not receive your W-9 back to us within 30 days from the original date of your account set-up, we will have to withhold 30% of your donated funds until we receive the form for our files.  This is a mandatory law mandated by the IRS, and We Are One Designs will submit the 30% to the IRS at the end of the year if no W-9 form is received.
